Importance: Cardiorespiratory fitness is an important marker of childhood health and low fitness levels are a risk factor for disease later in life. Levels of children's fitness have declined in recent decades. Whether school-based physical activity interventions can increase fitness at the population level remains unclear. Objective: To evaluate the effect of an internet-based intervention on children's cardiorespiratory fitness across a large number of schools. Design, Setting, and Participants: In this cluster randomized clinical trial, 22 government-funded elementary schools (from 137 providing consent) including 1188 students stratified from grades 3 and 4 in New South Wales, Australia, were randomized. The other schools received the intervention but were not included in the analysis. Eleven schools received the internet-based intervention and 11 received the control intervention. Recruitment and baseline testing began in 2016 and ended in 2017. Research assistants, blinded to treatment allocation, completed follow-up outcome assessments at 12 and 24 months. Data were analyzed from July to August 2020. Interventions: The internet-based intervention included standardized online learning for teachers and minimal in-person support from a project mentor (9-10 months). Main Outcomes and Measures: Multistage 20-m shuttle run test for cardiorespiratory fitness. Results: Of 1219 participants (49% girls; mean [SD] age, 8.85 [0.71] years) from 22 schools, 1188 students provided baseline primary outcome data. At 12 months, the number of 20-m shuttle runs increased by 3.32 laps (95% CI, 2.44-4.20 laps) in the intervention schools and 2.11 laps (95% CI, 1.38-2.85 laps) in the control schools (adjusted difference = 1.20 laps; 95% CI, 0.17-2.24 laps). By 24 months, the adjusted difference was 2.22 laps (95% CI, 0.89-3.55 laps). The cost per student was AUD33 (USD26). Conclusions and Relevance: In this study, a school-based intervention improved children's cardiorespiratory fitness when delivered in a large number of schools. The low cost and sustained effect over 24 months of the intervention suggests that it may have potential to be scaled at the population level. OBJECTIVES: To determine if subpopulations of students benefit equally from school-based physical activity interventions in terms of cardiorespiratory fitness and physical activity. To examine if physical activity intensity mediates improvements in cardiorespiratory fitness. DESIGN: Pooled analysis of individual participant data from controlled trials that assessed the impact of school-based physical activity interventions on cardiorespiratory fitness and device-measured physical activity. PARTICIPANTS: Data for 6621 children and adolescents aged 4-18 years from 20 trials were included. MAIN OUTCOME MEASURES: Peak oxygen consumption (VO2Peak mL/kg/min) and minutes of moderate and vigorous physical activity. RESULTS: Interventions modestly improved students' cardiorespiratory fitness by 0.47 mL/kg/min (95% CI 0.33 to 0.61), but the effects were not distributed equally across subpopulations. Girls and older students benefited less than boys and younger students, respectively. Students with lower levels of initial fitness, and those with higher levels of baseline physical activity benefitted more than those who were initially fitter and less active, respectively. Interventions had a modest positive effect on physical activity with approximately one additional minute per day of both moderate and vigorous physical activity. Changes in vigorous, but not moderate intensity, physical activity explained a small amount (~5%) of the intervention effect on cardiorespiratory fitness. CONCLUSIONS: Future interventions should include targeted strategies to address the needs of girls and older students. Interventions may also be improved by promoting more vigorous intensity physical activity. Interventions could mitigate declining youth cardiorespiratory fitness, increase physical activity and promote cardiovascular health if they can be delivered equitably and their effects sustained at the population level.

OBJECTIVES: To determine the impact of bushfires on children's physical activity. DESIGN: Natural experiment comparing device-measured physical activity and air quality index data for schools exposed and not exposed to the Australian bushfires. METHODS: Participants were drawn from 22 schools participating in a cluster randomised controlled trial of a school-based physical activity intervention that coincided with the 2019 Australian bushfires. Students in Years 3 and 4 (8-10 years old) provided data. We used propensity score matching to match 245 exposed and 344 control participants. MAIN OUTCOME MEASURES: Minutes of moderate and vigorous physical activity. RESULTS: The bushfires had minimal effect on children's average weekly physical activity. Analysis of acute effects showed children maintained their levels of physical activity up to an estimated turning point of air quality index of 737.08 (95% CI = 638.63, 835.53), beyond which daily physical activity levels dropped sharply. Similar results were found for girls and boys and for children from low-to-average and higher socio-economic backgrounds. CONCLUSIONS: Children's physical activity was not strongly influenced by the presence of smoke and targeted public health advice during the bushfires might not have had the intended effect of reducing children's outdoor physical activity. Only when air quality deteriorated to approximately 3.5 times the Air Quality index threshold (>200) deemed 'hazardous' by the Australian Department of Health did children's physical activity decline. Public health agencies should re-evaluate the effectiveness of health messages during bushfires and develop strategies to mitigate risks to children's health.

School-based physical education (PE) provides opportunities to accumulate moderate-to-vigorous physical activity (MVPA), but many students are insufficiently active during PE lessons. Providing teachers with feedback regarding their students' physical activity may increase the effectiveness of PE for achieving MVPA goals, but existing physical activity monitoring technologies have limitations in class environments. Therefore, the purpose of this study was to develop and validate a system capable of providing feedback on PE lesson MVPA. Equations for translating step counts to %MVPA were derived from measures in 492 students who concurrently wore an ActiGraph GT3X+ (ActiGraph) and Yamax pedometer (Yamax) during a PE lesson. To enhance feedback availability during PE lessons, we then developed a bespoke monitoring system using wireless tri-axial pedometers (HMM) and a smart device app. After developing and testing the monitoring system, we assessed its validity and reliability in 100 students during a PE lesson. There was a strong correlation of 0.896 between step counts and accelerometer-determined %MVPA and quantile regression equations showed good validity for translating step counts to %MVPA with a mean absolute difference of 5.3 (95% CI, 4.4-6.2). The physical activity monitoring system was effective at providing %MVPA during PE lessons with a mean difference of 1.6 ± 7.1 compared with accelerometer-determined %MVPA (7% difference between the two measurement methods). Teachers and students can use a smart device app and wireless pedometers to conveniently obtain feedback during PE lessons. Future studies should determine whether such technologies help teachers to increase physical activity during PE lessons.

Training and competition loads have emerged as valuable injury risk factors but very few studies have explored injury outcomes in adolescent athletes. The aims of this study were to describe injuries and to explore the relationship between training and match load volumes and injury in adolescent athletes participating in multiple contact team sports. One hundred and three male youth rugby athletes aged 14-16 years from 8 rugby union teams were prospectively monitored during a season for weekly training and match volumes and injuries. The relationship between volume and injury was explored by comparing the weekly volume in the week prior to an injury vs weeks without injury. There were 83 time-loss injuries in 58 athletes (62%). Overall injury incidence was 18.5 per 1000 player-hours. Mean weekly injury prevalence was 27% (95% CI 25-30). Average weekly volume was 5.4 (2.2) hours comprising 1.4 (1) match hours and 4 (2.6) training hours. Compared with weeks without injury, weeks prior to an injury had higher match volumes (110 [57] min vs 83 [59] min, P < 0.001). Poisson regression demonstrated that match volume was a predictor of injury with an odds ratio of 1.41 (P = 0.001). The contribution of match volumes to injury risk and the relatively high injury burden in these athletes may be profound. Very high match volumes are unlikely to be in the best interests of young athletes and could be avoided with a systematic approach to load management and athlete development.

A low-carbohydrate ketogenic diet reduces body mass without compromising performance in powerlifting and Olympic weightlifting athletes. J Strength Cond Res 32(12): 3382-3391, 2018-Weight class athletes use weight-making strategies to compete in specific weight categories with an optimum power-to-weight ratio. There is evidence that low carbohydrate diets might offer specific advantages for weight reduction without the negative impact on strength and power previously hypothesized to accompany carbohydrate restriction. Therefore, the purpose of this study was to determine whether a low-carbohydrate ketogenic diet (LCKD) could be used as a weight reduction strategy for athletes competing in the weight class sports of powerlifting and Olympic weightlifting. Fourteen intermediate to elite competitive lifting athletes (age 34 ± 10.5, n = 5 female) consumed an ad libitum usual diet (UD) (>250 g daily intake of carbohydrates) and an ad libitum LCKD (≤50 g or ≤10% daily intake of carbohydrates) in random order, each for 3 months in a crossover design. Lifting performance, body composition, resting metabolic rate, blood glucose, and blood electrolytes were measured at baseline, 3 months, and 6 months. The LCKD phase resulted in significantly lower body mass (-3.26 kg, p = 0.038) and lean mass (-2.26 kg, p = 0.016) compared with the UD phase. Lean mass losses were not reflected in lifting performances that were not different between dietary phases. No other differences in primary or secondary outcome measures were found between dietary phases. Weight class athletes consuming an ad libitum LCKD decreased body mass and achieved lifting performances that were comparable with their UD. Coaches and athletes should consider using an LCKD to achieve targeted weight reduction goals for weight class sports.

Given the importance of ensuring athletes train and compete in a nonfatigued state, reliable tests are required to regularly monitor fatigue. The purpose of this study was to investigate the reliability of a cycle ergometer to measure peak power during short maximal sprint cycle efforts in running-based team sport athletes. Fourteen professional male Australian rules footballers performed a sprint cycle protocol during 3 separate trials, with each trial separated by 7 days. The protocol consisted of a standardized warm-up, a maximal 6-second sprint cycle effort, a 1-minute active recovery, and a second maximal 6-second sprint cycle effort. Peak power was recorded as the highest power output of the 2 sprint cycle efforts. Absolute peak power (mean ± SD) was 1502 ± 202, 1498 ± 191, and 1495 ± 210 W for trials 1, 2, and 3, respectively. The mean coefficient of variation, intraclass correlation coefficient, and SE of measurement for peak power between trials was 3.0% (90% confidence intervals [CIs] = 2.5-3.8%), 0.96 (90% CIs = 0.91-0.98), and 39 W, respectively. The smallest worthwhile change for relative peak power was 6.0%, which equated to 1.03 W·kg⁻¹. The cycle ergometer sprint test protocol described in this study is highly reliable in elite Australian rules footballers and can be used to track meaningful changes in performance over time, making it a potentially useful fatigue-monitoring tool.

BACKGROUND: The expectation that training enhances performance is well explored in professional sport. However, the additional challenges of physical and cognitive maturation may require careful consideration when determining workloads to enhance performance in adolescents. OBJECTIVE: The objective of this study was to determine the state of knowledge on the relationship between workloads, physical performance, injury and/or illness in adolescent male football players. METHODS: A systematic review of workloads, physical performance, injury and illness in male adolescent football players was conducted. Studies for this review were identified through a systematic search of six electronic databases (Academic Search Complete, CINAHL, PsycINFO, PubMed, SPORTDiscus, and Web of Science). For the purpose of this review, load was defined as the cumulative amount of stress placed on an individual from multiple training sessions and games over a period of time, expressed in terms of either the external workloads performed (e.g., resistance lifted, kilometres run) or the internal response (e.g., heart rate, rating of perceived exertion) to that workload. RESULTS: A total of 2,081 studies were initially retrieved from the six databases, of which 892 were duplicates. After screening the titles, abstracts and full texts, we identified 23 articles meeting our criteria around adolescent football players, workloads, physical performance, injury and/or illness. Seventeen articles addressed the relationship between load and physical performance, four articles addressed the relationship between load and injury and two articles addressed both. A wide range of training modalities were employed to improve the physical performance of adolescent football players, with strength training, high-intensity interval training, dribbling and small-sided games training, and a combination of these modalities in addition to normal football training, resulting in improved performances on a wide range of physiological and skill assessments. Furthermore, there was some (limited) evidence that higher workloads may be associated with the development of better physical qualities, with one study demonstrating enhanced submaximal interval shuttle run performance with each additional hour of training or game play. Of the few studies examining negative consequences associated with workloads, increases in training load led to increases in injury rates, while longer training duration was associated with a greater incidence of illness. CONCLUSION: The combined capacity for adolescent males to grow, train and improve physical performance highlights and underscores an exciting responsiveness to training in the football environment. However, the capacity to train has some established barriers for adolescents experiencing high workloads, which could also result in negative consequences. Additional research on stage-appropriate training for adolescent male footballers is required in order to address the knowledge gaps and enhance safe and efficient training practices.

Player activity profiles of match play provide valuable information for optimal athlete training prescriptions, competition strategies, and managing load and recovery and are currently lacking in elite Australian-league (A-League) soccer. The aims of the study were, therefore, to (a) determine match activity profiles for elite A-League soccer players and make match-half and positional comparisons and (b) examine the effect of situational factors including evolving match status (drawing, winning, or losing) and goals being scored and conceded on selected match activity profile variables. Global positioning system tracking devices were used to determine activity profiles of 19 elite male adult soccer players during 8 preseason matches (n = 95 files). Total distance, average speed, high-intensity running (HIR) distance, and very high-intensity running distance decreased from the first to the second half by 7.92, 9.47, 10.10, and 10.99%, respectively. Midfielders covered 11.69% more total distance, 28.08% more HIR distance, and had a 10.93% higher average speed than defenders (p ≤ 0.05; d = 1.90, 1.03, and 1.83, respectively). Attackers performed 27.50 and 30.24% less medium accelerations than defenders and midfielders, respectively (p < 0.01; d = 1.54, and 1.73). When the team was winning, average speed was 4.17% lower than when the team was drawing (p ≤ 0.05, d = 0.32). Scoring or conceding goals did not appear to affect HIR. This study adds to limited knowledge of match demands in elite A-League soccer. The match activity profiles provide descriptive benchmarks that could be used to make comparisons with other elite level soccer populations while also providing a framework for game-specific training prescription, competition strategy, and load management. The generalization that defenders experience a relatively lower match load may be questionable given their relatively high acceleration and deceleration demands.

This research described the physiological demands of participation in adolescent rugby union including positional differences and the degree to which training practices replicate game demands. Between 2003 and 2008, 118 male adolescent rugby players aged 14 to 18 years were recruited from 10 teams representing 3 levels of adolescent rugby. Time-motion analyses using global positioning satellite tracking devices (SPI10; GPSports Systems Pty Ltd 2003) and computer-based tracking software (Trak Performance; Sports Tec Pty Ltd) applied to video footage determined player movement patterns 161 times during rugby training sessions and 53 times during rugby games. Compared with rugby training, rugby games were consistently characterized by more time spent jogging (14 vs. 8%), striding (3.2 vs. 1.3%), and sprinting (1.3 vs. 0.1%) (p < 0.001). Players also covered greater distances (4000 ± 500 vs. 2710 ± 770 m) and performed more sprints (21.8 vs. 1) during games compared with training (p < 0.001). The average sprint duration of 2 seconds was similar in games and training; however, the frequency of sprint efforts in training sessions was low (1 per hour). A major finding of this study is the disparity between physical game demands and on-field rugby training practices in adolescent players determined using time-motion analyses. Sprint pattern differences between games and training in particular could have important implications for player performance during competition. Results of this study should assist in the development of game-specific training sessions and drills that provide the kinds of physically demanding experiences observed in games. Additionally, coaches could assist in the management of adolescent players' participation loads by increasing the intensity and specificity and decreasing the volume of training.

Despite increased professionalization of adolescent sport and improved articulation to elite adult participation, the impact of sports such as rugby union among adolescents is under-explored. This study describes psychological stress-recovery responses relative to training loads in 106 male adolescent rugby union players. The results showed that players with the highest training and physical activity volumes during the season demonstrated more favourable recovery-stress states than moderate- and low-volume groups. Stress and under-recovery did not increase with increases in weekly volume when assessed across a season. When assessed more acutely during intensive competition phases, stress and under-recovery increased with increases in participation demands. Despite better psychological stress and recovery profiles of more elite, higher-load players, not all participants demonstrated favourable capacities to deal with stress and recovery processes. Seven participants were in at least two of three categories of highest volume, highest stress, and poorest recovery. Even in the absence of a full understanding of the impact of high-volume, high-stress, poor-recovery participation among adolescent athletes, these markers may be precursors for more deleterious outcomes such as injury, performance decrements, and overtraining. Findings support the efficacy of serially monitoring young athletes.

PURPOSE: Investigating adolescent training loads might help us understand optimal training adaptations. GPS tracking devices and training diaries were used to quantify weekly sport and other physical activity demands placed on adolescent rugby union players and profile typical rugby training sessions. METHODS: Participants were 75 males age 14 to 18 y who were recruited from rugby teams representing 3 levels of participation: schoolboy, national representative, and a selective sports school talent squad. RESULTS: Schoolboy players covered a distance of (mean +/- SD) 3511 +/- 836 m, representative-squad players 3576 +/- 956 m, and talent-squad players 2208 +/- 637 m per rugby training session. The representative squad recorded the highest weekly duration of sport and physical activity (515 +/- 222 min/wk), followed by the talent squad (421 +/- 211 min/week) and schoolboy group (370 +/- 135 min/wk). Profiles of individual players identified as group outliers showed participation in up to 3 games and up to 11 training sessions per week, with twice the weekly load of the team averages. CONCLUSION: Optimal participation and performance of adolescent rugby union players might be compromised by many high-load, high-impact training sessions and games and commitments to other sports and physical activities. An improved understanding of monitoring and quantifying load in adolescent athletes is needed to facilitate best-practice advice for player management and training prescription.

Despite widespread encouragement for children to participate in sport, the efficacy of early sporting pathways remains underexplored. We compared a rotational junior-sport model combining skills from rugby, cricket, and netball with a modified games model. Motion analysis was used to quantify movement. Results revealed no differences between sporting models in relative percent time spent stationary (p = .32), walking (p = .89), jogging (p = .45), and fast running (p = .06). The rotational model had a greater number of skill-development opportunities per minute (median = 3.4) compared with the modified games model (median = 1.1, p = .001). Promising results from varied and rotational skill exposure warrant further elucidation.
